Founded in 1982, Prospect Home Care & Hospice provides the highest quality home care and hospice services to the residents of the Ute Pass and neighboring areas. Home Care meets the needs of individuals recuperating under physician orders; Hospice meets the needs of individuals with a shortened life expectancy.

Our Home Care program deploys Registered Nurses, Physical Therapists, Occupational Therapists, Speech Language Pathologists and Certified Home Care Aides to implement physician orders for recovery and recuperation to attain the highest level of mobility and healing achievable. From wound care, catheter care, medical management, mobility training and therapies, our staff cares for nearly every diagnosis imaginable.

Our sophisticated Hospice Program deploys specially trained clinicians who are expert at symptom and pain management. Under physician orders, an interdisciplinary team of nurses, therapists, family service specialists and grief specialists integrate all needed services for maximum benefit to the patient and family. Hospice allows the patient to be in control, in their own home, with access to all resources available for a dignified life. Our Bereavement Team continues to support the family for a year after the departure of their loved one.

Our Helping Hands program assigns Homemakers and Personal Care Providers to homes where individuals are struggling to maintain their nutrition, activities of daily living, and personal care. The purpose of the program is to ensure frail, elderly and disabled remain independent and safe in their homes for as long as possible. Many of our clients receive government support for part of the cost of this service and Prospect Home Care & Hospice matches many of the program cost requirements through donations and grants. This program does not generate revenue enough to meet all expenses, but it is an essential lifeline to participants.

Our Helping Hands program also installs Personal Emergency Response Systems…the “button” summons the help and assistance needed when an emergency presents for individuals living alone. We have documented saving lives when a call is activated. Additionally, there is new technology available as part of the personal emergency response monitoring system to detect falls. The device is worn next to the body and activates to request emergency assistance in the event of a fall. There is great comfort for the client as well as for the family knowing that help is quickly available.

Over the years, we’ve built the three divisions – Home Care, Hospice and Helping Hands to provide access and continuum of health care to our community.
